Bailey HCA/240 August 21, 2011 Dr. Monica Reed, PhD, MPH Diabetes is a disease that affects millions of people every year, and in most cases, this is a disease that is preventable. Increasing public awareness regarding risk factors and how this debilitating disease can be controlled and prevented may reduce the number of cases diagnosed each year. Throughout the years, scientists have developed a variety of treatment options and medications that are available for individuals diagnosed with diabetes. Future treatments for diabetes may include an artificial pancreas and stem cell therapy (Yoffee, 2009). Along with community support through awareness programs and advances in technology and treatment options, this disease is controllable and in some cases, may even be prevented. Diabetes and who it Affects Most Diabetes mellitus (DM), more commonly known as diabetes, is the result of insufficient insulin production or the body’s inability to respond to insulin (Forth & Jude, 2011). Diabetes is a disease that affects how the body uses blood glucose, or blood sugar. Blood glucose is vital to health because it is an essential source of energy for cells of the muscles and other tissues as well as the brain’s fuel (Mayo Clinic, 2011). Individuals that suffer from diabetes have difficulty regulating and maintaining healthy blood glucose levels. If an individual is diagnosed with diabetes, no matter type, he or she has too much glucose. Some genetic factors do play a role in an individual’s risk for developing diabetes along with other factors such as race, but lifestyle is to blame for most diagnosed cases of diabetes (Centers for Disease Control and Prevention (CDC), 2011). Anyone who is 45 years or older should consider testing for diabetes, especially if he or she is overweight or has a family history of this disease. Race may also play a role in the development of this disease. Individuals who are African American, American Indian, Asian American, Pacific Islander, or Hispanic American/Latino should also consider prescreening (CDC, 2011). Another group of people that are more susceptible to DM2 are lower income groups, especially women (Lega, Ross, Zhong, & Dasgupta, 2011). Diabetes affects the whole body including the most important hormonal system, the endocrine system. The Body System Affected by Diabetes and its Causes The endocrine system is a group of organs or glands that secrete hormones into the bloodstream, which can affect the entire body. Glands of the endocrine system include the hypothalamus, pituitary, thyroid, parathyroid, pancreas, adrenal, testes, and the ovaries. The glands of the endocrine system release hormones into the bloodstream. The pancreas is the organ of the endocrine system, and when it is not functioning properly, the result is diabetes. Diabetes occurs when the pancreas malfunctions. One of the pancreas’s functions is to control the glucose level in the blood (Mulvihill, Zelman, Holdaway, Tompary, & Raymond, 2006). The pancreas secretes two hormones known as glucagon and insulin, which raise and lower glucose levels in the blood (Mulvihill et al. 2006). When insulin is secreted, blood glucose levels rise, and when glucagon is released, blood glucose levels decrease. If an individual is diagnosed with diabetes, his or her pancreas is not secreting enough insulin or target cells are not responding to the insulin that is being secreted (Mulvihill et al. , 2006). This malfunction of the pancreas results in either Type One (DM1) or Type Two (DM2) diabetes. DM1 diabetes occurs when the pancreas fails to produce insulin, and DM2 diabetes occurs when the pancreatic islets fail to secrete enough insulin or when target cells ignore the insulin (American Diabetes Association, 2011). The most common cause of diabetes is obesity, but other risk factors are involved as well. Risk Factors Some common risk factors for DM1 include genetics from both parents, environmental factors such as cold climates, viruses, race, and early dietary factors such as low vitamin D or exposure to cow’s milk (Mayo Clinic, 2011). Risk factors for DM2 consist of obesity, high or low birth weight, rapid weight gain in childhood, sedentary lifestyle, fetal exposure to gestational diabetes, family history, and race (Schub & Caple, 2010). Some of these risk factors may be controlled, whereas others cannot e controlled. Risk factors that are controllable include dietary needs and lifestyle choices. Obesity is the number one cause of DM2 among children and adolescents, and is commonly describes as body mass index, or BMI, which is a measurement of body fat that is based on height and weight (Schub & Caple, 2010). Obesity is a risk factor that is modifiable and may decrease an individualà ¢â‚¬â„¢s risk for developing DM2, and it may either prevent or reduce the onset. Obesity is a risk factor of DM1 as well. Although it may not prevent DM1, healthy lifestyle choices may help manage this type of diabetes as well. Other factors that cannot be controlled include genetics, fetal exposure to gestational diabetes, low birth rate, some environmental factors, and race. In today’s world, advances in medicine and technology have increased the availability of treatment options for patients diagnosed with diabetes compared to the past. Evolution of Treatment Options Over past several years, the list of medications used to treat diabetes has increased creating more options for patients and healthcare providers (Davis, 2011). In the past, insulin was the only treatment available for diabetes, and medications were limited. Metformin is an oral drug that has been used for many years as a treatment for DM2, but it is not always effective because of side effects such as gas, bloating, and diarrhea (Davis, 2011). With new advances in technology, this drug is now available in an extended-release formula, which reduces its side effects and makes is much more tolerable for many patients, and it is also available in a generic version, which is much less expensive for numerous individuals suffering from DM2 (Davis, 2011). Advances in treatment options have also led to the availability of drugs such as Byetta, Januvia, Onglyza, Tradjenta, and Symlin (CDC, 2011). Januvia, Onglyza, and Tradjenta are drugs that work with natural enzymes and the body’s own insulin to control blood glucose, Symlin is a synthetic hormone that helps lower blood glucose after meals, and Byetta is an injectable drug that assaults high blood sugar and diminishes one’s appetite (Davis, 2010). Diagnosis and treatments depend on which type of diabetes and individual may have. A healthcare professional will conduct different types of medical tests and procedures to determine the proper diagnosis. Current Treatment Methods When a patient suspects that he or she may have diabetes, the first thing they should do is see their doctor. The physician can perform tests and procedures that will determine whether or not he or she has diabetes, and if so, which type they have. These tests may include a fasting blood glucose level test, random (non-fasting) blood glucose level test, oral glucose tolerance test, hemoglobin A1c test, or a ketone test, which is used to diagnose DM1 (Right Health, 2010). A fasting blood glucose level test is a test that indicates a patient’s blood glucose level after a period of eight hours of fasting. At the doctor’s office, a medical professional will take a sample of blood from the vein in the inner bend of the elbow. The fasting blood glucose level should be below 99mg/dl, which is considered normal. Results that indicate levels of 100 to 125 are indications of pre-diabetes and 126 or higher indicate diabetes (CDC, 2011). Another test is random blood glucose level. Random blood glucose level test is a blood glucose test conducted on a patient that has not been fasting. Normal levels in and average adult are 70 to 130 mg/dl (CDC, 2011). If an individual’s test results reflect a level of 130 to 200, this may be an indication of pre-diabetes, and a patient with a level above 200 is considered a diabetic (CDC, 2011). A glucose tolerance test may also be used to diagnose diabetes. A glucose tolerance test is in which the patient, who has been fasting for about 10 hours, drinks a solution of glucose. After the patient drinks the solution, his or her blood and urine is sample are taken and analyzed over a period of three hours (Mulvihill, et al. , 2006). The urine test shows the presence or absence of glucose in the urine, or ketones (Mulvihill, et al. 2006). The A1c test is a test used for diabetics as well. An A1c is a test which indicates blood sugar levels over a period of three months (Medicine Net, 2011). This is a test that benefits the patient and the physician by giving an indication of how well a medication may be working. Normal range is 4-5. 9%, a level of 8% or more is an indication of poorly controlle d diabetes (Medicine Net, 2009). With advancing technology and medicine, the future holds more treatment options. Future Treatment Options Advancement in technology is making new treatment options available for many diabetic patients. Research has indicated new treatments such as an artificial pancreas and pancreatic cell transplantation (Yoffee, 2009). Medtronic, a medical device company, is combining glucose monitoring systems with insulin pumps to recreate the delivery system of insulin in the body (Yoffee, 2009). Scientists are also working on transferring pancreatic cells from pigs into humans (Yoffee, 2009). Even though advances in technology are allowing scientists and other professionals to invent new and exciting treatments for diabetes, prevention is the best defense against this disease. Evaluation of an Existing Educational Program One educational program that has made great strides to combat the growing threat of diabetes among children and adolescents is the Adolescent Health and Youth Development (AHYD) Unit of the Georgia Division of Public Health. They have helped fund 30 Teen Center Programs in 28 counties in the state of Georgia (Georgia Division of Public Health, 2011). One of these programs is the Diabetes Prevention and Control Program (DPCP), which was established to reduce the burden of diabetes on the communities of Georgia. This program provides communication to increase awareness, improve quality of care, increase screening, and reduce costs (Georgia Division of Public Health, 2011). This program has made some impact on communities, but there is still a lot of work that needs to be done. The DPCP was recognized by the CDC for achievement in A1c screening and improving the well-being of people affected by diabetes. Scholarships have been awarded to 374 medical professionals that help them pay for the Certified Diabetes Educators (CDE) exam. These are great accomplishments, but it is not enough. More needs to be done to reach out to the communities that are hardest hit by this disease. In addition to the A1c screening, a lifestyle initiative should be added. Adding a lifestyle initiative that would include local restaurants and other businesses to come out into their communities and get involved. Six annual picnics that included A1c screening, lunch, activities, and educational materials would be beneficial for many communities throughout Georgia. Local businesses could ban together and donated good to raffle off, which would draw families and community members to the event. The event could be held at a different area parks around the state. The hardest hit areas would be the first on the list. People need to be educated about the risks, causes, and prevention of diabetes, and our communities must come together in order to combat the onset of diabetes in our children and adolescents. An Effective way of Spreading the News No program will be successful without the proper advertising or spokesperson. In today’s world children and adolescents are drawn to Sponge Bob. If he were the spokesperson for healthy eating and exercise, children would react with enthusiasm and excitement. His picture on fliers that promote a healthy lifestyle could be mailed to surrounding area residents alerting them of the upcoming events. Children could come to the event and get their picture taken with Sponge Bob. Sponge Bob could lead a relay race for children and adolescents. An ad in the local newspaper would benefit the event as well. A website could be created for this event with links to Facebook and Twitter. With the help of Sponge Bob and the community, education on the benefits of a healthy lifestyle would reach a greater number of people, and help promote the prevention of diabetes. Conclusion Diabetes is a disease that affects millions of people. In some cases, this disease can be prevented by teaching and promoting healthy lifestyle choices. Over the years, many treatment options have changed, and with increasing advances in technology and medicine, there will be more changes in the future. So therefore the right to bear arms has been taken advantage of and as lead to many malicious events like The Columbine Shootings, Virginia Tech, Aurora Shooting, and New Town Shootings. In 1999, a failed bombing which turned into a shooting occurred at Columbine High School. This event is known as the Columbine Shootings in which two high school seniors, by the name of Eric Harris and Dylan Klebold, were involved. According to an article â€Å"7 Myths about the Columbine Shooting† by Trevin Wax, both convicts seemed like regular teenagers that, â€Å"attended football games, dances, and school plays†¦ [they were not] part of a street gang or known to dress in gothic style,† but they had been arrested a couple of months before for stealing. They also seemed to be part of the Trench Coat Mafia, which were a group of students dressed in black coats that were against school bullies. Since Eric and Dylan were targets of bullying, they both decided on a plan to get revenge on the people who were mean to them last year. So for that whole year, both teenagers gathered up materials to carry out their plan. They had been researching, building pipe bombs, and purchasing guns from their older friends who were unaware of what the consequences could lead to. Their plan for revenge was to blow up Columbine’s cafeteria by placing â€Å"explosives to kill about 100 people. † However that was not enough for their plan. They decided that once the explosives go off inside the school, they would be waiting outside and shoot the remaining students and teachers that would come out of the school, trying to escape. However, everything didn’t go as they expected. The bombs that they had placed inside school never went off, so therefore Eric and Dylan just decided to go in and shoot whoever they could. They had no specific range; everyone was a target to them. After killing twelve students, one teacher, and injuring 24 others, Eric and Dylan pointed their weapons towards hemselves and committed suicide. They were both found dead on the floor, fully armed, and with their explosives around them. According to Eric and Dylan’s journals, they didn’t expect to come out of there alive. They knew they would end up dead from either police bullets or by their own hand. So committing suicide or ending up dead was part of their risky plan. Because Eric and Dylan had such high temper, they felt as if they needed to get revenge on everybody who ever brought their confidence down by killing them, and illegal weapons were their first reaction. Even though teenagers aren’t allowed to purchase guns, Eric and Dylan had access to them due to irresponsible private gun sellers. Now a day, people are trying to argue that placing restrictions on the 2nd amendment is against the US Constitution and that it’s violating their rights. However risky behaviors and actions, like the Columbine Shootings have shown that restrictions over gun control are needed for safety reasons due to the fact that it has gotten out of control. Society, in the few previous years, has shown how they cannot be trusted with illegal weapons. If every single person owned a gun in the US, then this country would not be a safe place to live in. The Virginia Tech shooting is known as the deadliest mass murder by single gunmen. The event happened as follows: The first killings began at 7: 15 a. . in West Ambler Johnston Residence Hall, two people in the same dorm as Cho, were the victims. At 9:26, more than two hours after the first killings began did the school decide to notify the students via email that the shootings had occurred. By that time it was too late for the students and teachers having class to escape the fury of Cho. Cho chained the doors shut in the building known as Norris Hall; he also decided to leave a bomb threat if anyone decides to come in. The shootings took place on the second floor of the building. The second shooting left 30 people left and over fifteen injured. The victims were caught off guard and killed by a senseless act of evil. After the damage had been done the police break into Norris Hall, police officers not only find the bodies of the victims but they also find the body of the perpetrator. Cho had taken his own life after noticing that the officers were coming into the building. With the killer dead many questions of why the massacre took place were left unanswered. As it turns out Cho mailed a package to the NBC network between the first and the second shooting. The package contained many irrelevant towards the massacre that is except one letter. That letter basically said that he was tired of the rich kids around him; he blames them for what took place. Cho also compares himself as being like Eric and Dylan from the Columbine Shootings. Ironically like Eric and Dylan Cho also went through bullying as a teen himself. In fact Cho was considered mentally ill, but when applying to Virginia Tech he did not inform the school about his special needs. Virginia Tech is a perfect of how The Second Amendment has failed to do what it was intended to do. The Second Amendment has allowed almost anyone to be in possession of a gun and in return has caused deadly massacres to ccur. It is obvious that Cho was mentally ill yet he was not denied the right to have a gun. The second Amendment was made to fit the time period of 1791 and it should respectively be altered to today’s day and time. It is not fair to the public to be on guard all of the time in case of another mass shooting. There should be extensive background checks in order for a person to come in contact with a gun. No mentally ill person or any criminal should be allowed to be in possession of any firearms for the safety of everyone around them. The Second Amendment should not be omitted but it definitely needs to be altered in order to prevent any extensive damage than what has already been done. Gun Control laws are desperately needed to stop these crimes from taking over. In 2012 the Aurora and Newtown Shootings, to name a few, have taken the life of innocent people. The Second Amendment has done the opposite of what it was originally meant to do, which was just for defense. How many more Newtown’s and Columbines need to happen in order to see the reality. The NRA (National Rifle Association) needs to be aware of the intensity of the situations that are taking place. Another example of why The Second Amendment has failed to do what it was intended to do is the incident now known as the Aurora shooting. On July 20, 2012, a mass shooting occurred at a movie theater during a midnight screening of the dark knight rise, in Aurora, Colorado. Killed 12 people and leaving 58 injured. The police identified him James Eagan Holmes, as the shooter. It is said that he bought a ticket and entered the theater. He sat in the front row and after 20 min into the film he exited the building through an emergency exit door and kept it slightly open with a rock. Then to return 30 min later into the film around 12:38, the police say, from his car that was parked near the exit door, he changed into protective gear wearing a gas mask. The audience didn’t at first considered him a threat because they assumed he was wearing a costume also like the others that dress up for the screening. Some also thought that he was playing a prank or he was part of a special effects installation set up for the films premiere as a publicity stunt. The gunman then threw a canister releasing a gas obscuring the audiences’ vision. He then started to fire a shot gun, semi-automatic rifle and finally a Glock 22 handgun at the audience, first at the back, then toward the people in the aisles. Around 12:45 am, police detained Holmes behind the cinema next to his car. He even told the police that his apartment was booby-trapped with explosive devices, so if anyone tried to open his door, the entire apartment would explode. James Holmes was described by those that knew him as a doctoral student, who was quiet and responsible; an image that confuses us with the man that opened fire at the movie theater. Holmes was a scholarship student who attended the University of California, Riverside, in 2006 and graduated with highest honors with a bachelor’s degree in 2010, later enrolling at University of Colorado School of medicine, as a doctoral candidate. The university police state that they have no record of having contact with him but only for a speedy summons. The biggest mystery around the 24-year-old doctoral student was why he did it, as a police allege. Holmes didn’t appear to have any social media contact. A along time neighbor told the authorities that he was to believe that he were just a â€Å"normal kid†. â€Å"You would never guess he was a violent guy,† Mitchell said describing Holmes as a â€Å"nerdish† and â€Å"a book-smart type guy. † Lynne Fenton, a psychiatrist who treated accused Colorado theater gunman James Holmes, stated that Holmes made a remark on June 11, about fantasizing about killing â€Å"a lot of people,† six weeks before the shooting. He rejected the offer to be confined for 72 hours. No one knew that he was actually mentally ill other than the psychiatrist that treated him. After the shooting the Colorado gun sales spiked, with number of background checks for those who seek to purchase a firearm, the increased also occurred in Washington, Florida, California and Georgia. He then makes his way into a first grade class where there was a sub and although she attempted to hide the children he killed all of the students besides a six year old girl. It has been told that the only way the little girl made it thru the shooting was playing died and stayed still till she heard no nose. The gunman then went into another first grade class with a teacher by the name of Victoria told the gun man that her kid where in the auditorium. Many of the kids ran out of the hiding place trying to escape the room and Lanza the shot them dead. Out of that class only six children were able to escape. Many of the teachers and faculty along with kids had a near death experience. One of the teachers saw two kids in the hallway on their way to the office and pulled those kids to safety. Along with the Liberian helping saved fourteen kids life by putting them in a lock that had a door that no one could get in without the lock. Although most of the kids and faculty was in first grades this sad day took a toll on not only the school, city but the whole nation. After this many students did not feel safe going back to school as parents were scared to send them back. The debate over whether to enhance restrictions on firearms has become a contentious focal point of state legislative sessions around the country this year. The Aurora shooting is a perfect example on how restrictions on gun control would have helped prevent anything related to that matter from ever happening. Yes although the second amendment states we have the right to bear there should be gun control laws that prevent any mentally ill person from purchasing any fire arms. If that had taken place Virginia Tech, Newtown and the Aurora Theater shooting would have been prevented.

Stylistic Techniques In Batman Returns Film Studies Essay Tim Burton is known for his obscure and unconventional methods of film-making. His stylistic techniques, influenced by German Expressionism, are very unique. For this reason I have chosen to look at a short clip from Tim Burtons Batman Returns. Batman Returns was produced in 1991 and released in 1992. When Selina Kyle, the receptionist of Max Shreck, discovers some crooked business plans, she is thrown from the top of a building by her corrupt boss, provoking her transformation into the mysterious and mischievous character of Catwoman. The scene in which this happens is very significant to the overall narrative of the film and to the development of the character of Selina Kyle. As Selina lies in the snow after being thrown from a building, there is a cold and harsh atmosphere as many cats surround her and nibble on her hands. The bright red colour of her blood that Burton uses is a contrast to the colour of the rest of the scene, such as the grey buildings and white snow. It stands out very noticeably and is therefore more striking to the audience, creating an even greater sense of unease. As Selina enters her apartment it is clear to the audience that she is not in a sound state of mind. Selina starts to knock things over violently in her home and she starts to shred away any evidence of the woman she used to be. There is a clear sense of irony in terms of the mise-en-scà ¨ne in this scene. All of the walls in her apartment are painted pink. This gives off a sense of girlishness and innocence. These are characteristics of Selinas that will no longer be relevant to her personality. The grotesque walls and numerous stuffed toys are effective visual devices used by Tim Burton as a method of narrative. This particular use of mise-en-scà ¨ne allows the audience to see Selinas old personality and her naivety before it is all stripped away. The fact that Tim Burton chose to create a feminine and controlled setting for Selinas apartment makes her deterioration a lot more effective. Her violent behaviour and the use of props, for example, when she dishevels her stuffed toys with a kitchen knife, is a premonition of her violent behaviour which is to come. The use of this prop makes the scene a lot more shocking. Cat imagery is very evident in this scene. Not only does Selinas home become invaded by alley cats, but she also adopts the characteristics of a cat as she pours milk into her mouth carelessly and licks her lips in an animalistic manner. This truly shows her changing state of mind as the old Selina would never behave in this way. The use of contrasting colours in this scene is also very evident. Burton chooses to contrast the girlish ly pink walls with black paint which is frantically sprayed along them by a manic Selina. Through his use of visual styles and the new setting of Selinas apartment, Tim Burton has created an evil, sinister and vengeful atmosphere which coincides with Selinas creation of the evil, sinister and vengeful character, Catwoman. The lighting at the beginning of this scene, as Selina lies in the snow, is low-key and Tim Burton creates a lot of shadows, which are very reflective of his expressionist influences. Like the beginning of the scene, the lighting is also low-key in Selinas apartment. During some of the frames in this scene the lighting is placed beneath Selinas face, creating shadows which give off a sense of insincerity. There are large shadows as she charges about her apartment trashing it piece by piece with exaggerated movements creating a frightening atmosphere. The low-key lighting in this scene is further emphasised when Selina breaks the light bulbs in her home. Here I think Tim Burton is extremely successful in portraying an evil and villainous atmosphere. As she smashes the light bulbs of her neon-lit sign she transforms it from a welcoming message which used to read Hello There to an uninviting and menacing message which now reads Hell Here. The lighting in this scene becomes even dimmer t han before, signifying the completion of Selinas transformation into Catwoman and unleashing an even more hellish setting. The editing at the beginning of this scene is very slow paced. As Selina strolls into her apartment in a state of shock there is a continuous long shot. The camera pans and tracks across the apartment, following Selina around. The slow editing evokes a state of calm. As the scene progresses and Selina becomes more and more infuriated the editing becomes a lot more rapid. There are a lot more cuts and there is a greater variety of camera shots. The audience are presented with many unsettling shots from extreme close ups of Selinas face as she licks her lips frantically to close ups of her stuffed toys in her kitchen sink being destroyed in an act of aggression. These shots are excellent in portraying a sense of panic among the audience. Burton uses point of view shots as Selina begins to construct her Catwoman attire. This makes the audience feel like they are a part of the action and is successful in provoking sympathy and empathy as the audience try to relate to the feelings that Se lina is having. Much the same as the editing, the sound in the beginning of this scene is a lot more calm and slow paced until the scene reaches its climax and the music becomes a lot more frantic and loud. The use of strings is clear in this scene. The music is very low in volume to begin with and is very high pitched which gives a very eerie tone to it. There is a lot of suspense created and the whining sounds of the strings convey a feeling of tension. There is also diagetic sound in this scene such as the creaking of the door and the cat meowing. As Selina destroys her home the music becomes a lot louder and fast paced. The pitch of the music becomes low and threatening. This symbolises Selinas new found control over her life and foresees the threat that she is about to bring onto the city of Gotham. Tim Burton uses all elements of film language in this scene and throughout the film effectively to achieve his desired aims and emotions for the audience. In this scene he is very successful in showing a complete alteration from the quiet, reserved character of Selina Kyle to the dominant, sexual and ruthless character of Catwoman.

My personal choice for order of dealing with the issues would be done so by my deciding which issues were more crucial and which ones needed to be dealt with first.With the list that was provided the order for these issues to be dealt with would take place in a specific order in order to promote the best chances of security and safety for all of the citizens involved. The first thing that would be done is that the airways would be secured and all incoming and outgoing air traffic would be stopped for some time. The next thing to do would be to secure waterways. This would include making sure that there were security efforts involved in protecting the city’s water sources as well.This woul d include the stopping of all water traffic and the security of the different ports and waterways that are surrounding the city. Next transport would be secured and stopped in order to allow for a more smooth evacuation of the city if necessary. This would mean that there need to be an exit strategy plan in the disaster plan that is on location at the town hall and other area disaster planning sites like the American Red Cross. All areas that produced power would be secured by as many forces as could be spared as well as trained disaster volunteers.This would be done so in order to help reduce the chances of harmful chemicals or toxins being released into the air in these areas. All food sources would be secured and there would be limited access to these sources in order to reduce the likelihood of contamination. Schools and churches would be looked at as possible places for there to be shelters and food banks would be contacted as part of the process of having food and other goods.Other places that would be contacted are local grocery stores and other discount stores that would have access to high quantity of needed supplies. This would be done in partial efforts on putting limits on what each person could buy or have in order to ensure that supplies lasted for the most people possible.
